Patient Forms

Due to new regulations mandated by the Federal Trade Commission to deter identity theft, patients must show two forms of identification at each appointment. Acceptable forms of identification include photo ID (drivers license or other), current medical insurance card(s), and correspondence showing current address (such as a utility bill). If your photo ID does not show your address, you must bring something that does. Failure to provide proper identification may delay the process to obtain a timely appointment with the physician. Thank you for your cooperation with this federal mandate.

Prescription Refills

Insurance Coverage

If your insurance company does not approve the prescribed medication, we will send information to the insurance company stating the reason the initially prescribed medication is necessary and ask that an exception be made. Approval of the exception is not guaranteed and may take several weeks.

Controlled Substances

A controlled substance is a medication whose use is regulated by state and federal law. Patients on controlled substances will need to obtain a written prescription (without refills) on a monthly basis. How do I schedule future appointments?

Throughout the month of June, you can continue to use our regular scheduling number: 763-452-3955. Starting July 1, please use our new scheduling number: 612-262-7100.

Will my care be covered by my insurance?

Allina Health is in-network for many insurance plans. However, it is a good idea to contact your insurance carrier to confirm coverage.
