Patient Forms
Due to new regulations mandated by the Federal Trade Commission to deter identity theft, patients must show two forms of identification at each appointment. If your photo ID does not show your address, you must bring something that does. Failure to provide proper identification may delay the process to obtain a timely appointment with the physician.
